Teams submitted must be comprised of individuals 18 years of age or younger.

One entry allowed per team. One team will be chosen and recognized per month.

All entries must be received by the 25th of each month in order to be considered for that month's award.
The team recognized will receive a pizza prize pack to Topper's Pizza, compliments of Furniture Superstore. Prize is not redeemable for trade or cash. 

Winners will be notified by email or phone call. A representative of the team selected will receive instructions on how to receive their prize at time of selection.

Each winner is responsible for all federal, state and local taxes and will be required to complete an IRS form W-9. Sponsor reserves the right to withhold prizes until completed form W-9 is received.
